Do you have any mutual friends that you could try to reach her through or ask if they have had any similarly weird experiences with her. She sounds like there’s something else going on with her right now. She’s probably hurting and could use some help. It would be easy to cut off contact and blame her, but it sounds like your relationship is a strong and long lasting one. She doesn’t seem like she’s in the right place to be the one to put in the effort right now to keep your friendship going, it’s gonna have to be you right now.

To be charitable to op that isn’t really what’s he seems to be suggesting anyway. He says it’s not really about the money and more about the thought. It sounds like he doesn’t want to have a “sugar daddy” style relationship, but feels like that’s what it’s becoming.
I think they need to have an open discussion about money. They need to figure out What each of their needs and expectations are around it. Trying to figure out non-monetary ways she can contribute to their lives, or take care of op might make the relationship feel more egalitarian.
Stop doing his laundry, stop cooking for him. If he doesn't change, stop living with him. Don't accept this labour division AT ALL. If he wants a housewife he can go find one.
There is something missing here. The symptoms you describe are extremely severe – having to shower her etc. How can there be no diagnosis? What exactly are her issues?
First and foremost she needs a diagnosis and more external help. Maybe even psychiatric help if doctors claim there is nothing wrong.
At the end of the day if you can't cope, you don't have to stay with her but it may be morally worth getting social workers involved if she genuinely cannot care for herself.
